Women considering breast augmentation, or those who already have breast implants, may have read or heard about Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL). BIA-ALCL is a rare illness linked to certain implants, which have been the subject of many news stories and discussions in the news and social media. The condition has been researched extensively and Dr. Mathew Mosher wants to ensure our patients are fully informed about the remote risk of BIA-ALCL and breast implant illness (BII) when considering breast augmentation.

Breast Augmentation With Fat Transfer

Instead of using breast implants to enlarge the breasts, Dr. Moser can perform “natural breast augmentation” using fat grafting or fat transfer. This procedure involves harvesting unwanted fat from one area of the body using liposuction and transferring it to the breasts. Fat transfer is best for providing a modest size increase or improving the shape of the breasts with or without implants. Transferring large volumes of fat isn’t predictable, so patients who want a significant increase in the size of their breasts are typically not good candidates for breast fat grafting.

Implant Material

Saline implants

These implants are filled with a medical-grade saltwater solution that is harmlessly absorbed by the body if leakage or rupture occurs. Since saline implants are filled after placement, they can be inserted through more incision locations and their final volume adjusted in the operating room to achieve optimal results. Despite these features, saline implants have become less popular choices in recent years for a variety of reasons. Generally, when compared to silicone gel implants, patients find that saline implants feel and move less like natural breast tissue. They have also been associated with a higher rate of revisional surgery attributable to factors such as more frequent problems of implant shell ripples and wrinkles.

Silicone gel filled implants

Since Health Canada approved silicone gel implants in 2001, they have rapidly gained popularity among breast augmentation patients. The gel filling in these implants has different levels of cohesiveness, and the patient’s choice is based on personal preference for the look and feel of the implant.

Implant Shape & Profile

We educate patients considering breast augmentation surgery about available implant shapes and profiles. Anatomical or contoured implant shapes are rarely recommended but are available for particularly challenging breast shape asymmetries. The most commonly recommended shapes are round implants. When appropriately selected to match the anatomical features of each patient, a round implant can achieve natural-looking results with more filling of the central and upper breast contours.

Implant Options

The implant profile refers to the projection of the implant from a profile view. Different profile options include:

  • Low

Incision Location Options

Dr. Mosher chooses from the following incision techniques when performing breast augmentation:

  • Inframammary incision: a 3 to 5 cm incision placed along the natural crease under the breast
  • Periareolar incision: a 4 to 6 cm incision placed along the bottom edge of the areola (the dark skin surrounding the nipple)

Implant Locations

After making the incision, Dr. Mosher inserts the implant into the breast pocket. These options include:

  • Behind the breast tissue (subglandular)
  • Behind the chest muscle (submuscular)

One method isn’t necessarily better than the other, but your best option will depend on a variety of factors, which Dr. Mosher can help you determine.

Rapid Recovery Breast Augmentation

Dr. Mosher uses careful surgical techniques and specialized instruments that minimize tissue trauma, resulting in less bruising and bleeding for a much more comfortable and rapid recovery. Our unique program for rapid recovery following breast augmentation allows many of our patients to go out for dinner in the evening following their surgery.

Recovery Timeline

Even though each patient recovers at her own pace, here is a general timeline for what you can expect:

Day of surgery: Expect the breasts to sit very high on the chest and feel tight and firm. There is mild to moderate bruising and swelling. You’ll feel more sore if the implants were placed under the chest muscle. Expect some swelling and discomfort initially; these can be managed with oral medications, stretching exercises, and cool compresses. We recommend sleeping on your back with your upper body slightly elevated for at least 2 weeks.

Days 2 to 5: You can shower 24 hours after the surgery. Avoid bending over or doing anything that increases tension on your incisions. Patients often return to non-physical jobs within 5 days of the procedure. You will wear a snug-fitting sports bra for approximately 2 weeks to provide support and to help with healing.

Weeks 2 to 6: During this time, any pain, discomfort, or soreness should be significantly abated. At this juncture, you can resume most of your regular physical activities, except high-impact activity or upper body exercises. Continue wearing a support bra or a sports bra to ensure that your breasts are supported. Strenuous exercise or weightlifting must be avoided for at least 4 weeks.

Week 6 and beyond: At about 6 weeks after surgery, implants usually start to “drop-and-fluff.” It will take about 3 months for the implants to settle and the final results to appear.

Are breast augmentations painful?

Dr. Mosher minimizes post-op discomfort significantly through his refined surgical techniques that limit stretching and trauma to surrounding tissues. Many of his breast augmentation patients even feel up to going out for dinner the evening following their surgery because of our proprietary Recipe for Recovery™ protocol.

Do I need to wear a special bra following surgery?

Yes, you will need to wear a special compression bra to support your breasts and minimize swelling for at least 2 weeks after surgery.

When will my implants drop?

It takes about 2 to 3 months for the chest muscles, breast skin and tissues to relax, allowing the implants to drop and settle into their final placement and shape.

Is the surgery safe?

Can I breastfeed after a breast augmentation?

How long after breast augmentation will I see results?

Your breasts will look noticeably larger right away, but it will take up to 3 months for the tissues to relax and for the breasts to “settle” into a more natural shape.

Will my implants last forever?

While today’s breast implants are more durable than ever, they aren’t made to last a lifetime. Most do last at least 10 years and don’t need to be replaced if there are no concerns or complications.
